An agreed direction can still leave a team unsure what happens next. The missing piece is often a decision that has not been named clearly: which choice is needed, who can make it and what information would make it easier.
Keep a short record of decisions alongside the work. Include the question, the person responsible, the available options and the point when the decision is needed. A list of actions alone may hide the choices that those actions depend on.
When a decision is made, record the reason as well as the outcome. This helps the team return to the thinking when circumstances change. The aim is not more paperwork; it is enough shared context to keep useful work moving without repeatedly starting the conversation again.
